Once you are comfortable and relaxed, your physician will place the ultrasound transducer into the vagina. A guide attached to the transducer leads the needle through the wall of the vagina and into each follicle in the ovaries. Your physician will collect the oocytes and follicular fluid into a test tube for transport to the Embryology lab. The laboratory staff will examine the oocytes microscopically. After the retrieval, you will be taken to a recovery room where you will be observed for 1-2 hours while the intravenous medications wear off. When you are fully awake, your vital signs are stable, and you have urinated, you will be released to go home. You may have some vaginal spotting and lower abdominal discomfort for several days following this procedure. Generally, patients feel completely recovered within 1-2 days. You should notify your physician immediately if you develop severe pain, heavy bleeding, or fever after the retrieval. The number of oocytes retrieved is related to the number of ovaries present, their accessibility, and the number of follicles that develop in response to stimulation. Ultrasound provides only an approximation of the number of oocytes that one can expect to recover. On the average, 8-15 oocytes are retrieved per patient. More than 95% of retrievals result in the recovery of at least one oocyte. Step 8 - Insemination of Oocytes The Embryology laboratory staff examines the fluid aspirated from follicles for the presence of oocytes. We routinely aspirate all mature follicles in order to obtain as many oocytes as possible. Not every follicle contains an oocyte, and rarely, a follicle may contain more than one. It is important to determine the maturity of the oocytes in order to time the insemination properly. The oocyte can only be fertilized during a short interval of about 12-24 hours. If the oocyte is either immature or postmature too old ; , it may not be capab1e of fertilization or normal development. If immature oocytes are obtained at retrieval, they can sometimes be matured in the laboratory prior to insemination. Normal pregnancies have occurred with such oocytes. Semen is usually collected by masturbation the morning of the retrieval. The staff will instruct you regarding time of collection and transportation to the office. On rare occasions, the laboratory staff may request a second semen sample . You must notify the staff beforehand if you are planning to leave town or will otherwise be unavailable after the first collection. We recognize the pressure that semen collection may generate under these circumstances. In many cases, some flexibility in the timing and even in the method of collection is possible. We also recommend semen cryopreservation freezing ; before oocyte retrieval as a backup or sometimes as the primary sperm source, for instance, ciprofloxacin.

| Reconstituted ceclor is stable in the refrigerator forBy Steve Edelson & Shaun Brown Senior Writers JPMorgan's 23rd Annual Healthcare Conference last week could be summed up in two words: rational optimism. Rational because companies and investors both agree that valuations aren't going to spiral upward out of control anytime soon. Optimism because companies with good stories are still likely to find their stock prices spiraling up. The theme is playing out both in the public and private spaces. First, the muted stock moves of public companies that presented at the conference could suggest the group is fairly valued see "JPMorgan Effect" ; . But there's also the conviction that money is available, as a quartet of companies filed for followons.

Products. Revenue from Ceckor CD declined by $26.0 million in 2001, from $39.4 million in 2000 to $13.4 million in 2001. Naprelan has been written down due to lower than forecast revenues in 2001 and reduced projected revenue and profitability from this product. The level of promotional support for a product can have a significant impact on the level of revenue generated from that product. Elan does not expect to provide any significant promotional support for Naprelan in the future and this has been reflected in the projections for this product. Revenue from Naprelan declined by $33.6 million in 2001, from $41.8 million in 2000 to $8.2 million in 2001. The product and acquired IP intangible write-downs discussed above are included in exceptional selling, general and administrative costs. Other exceptional selling, general and administrative costs were $74.4 million. These primarily relate to severance, integration, relocation and similar costs and asset write-downs arising from the integration of Elan's US Biopharmaceuticals business. Exceptional research and development costs were $78.6 million. These mainly relate to severance, integration and similar costs and asset write-downs arising from the closure or scaling back of various drug delivery programmes and sites. Elan's pulmonary drug delivery assets are being reorganised. Also included were costs of certain research programmes that Elan does not intend to complete. These were the costs incurred pending closure or sale. Exceptional net interest costs were $6.8 million. These mainly relate to costs associated with the redemption in March 2001 of the 4.75% Exchangeable Notes issued by Athena Neurosciences, Inc. "Athena" ; . In 2000, Elan incurred exceptional charges of $113.6 million. In November 2000, the FDA requested that the pharmaceutical industry voluntarily cease the distribution and marketing of products containing phenylpropanolamine "PPA" ; . The Company ceased shipment of the products and withdrew them from customers' warehouses and retail shelves. In connection with the termination of this activity, Elan incurred an exceptional charge of $35.6 million, primarily for product returns and the write-off of inventory and product intangible assets. Elan incurred charges of $0.6 million arising from the acquisition of Dura. Elan incurred charges of $10.4 million arising from the termination of certain research and development projects and charges of $21.4 million relating to the write-down of certain intangible assets arising from a change in focus of Elan's business. Elan incurred charges of $22.2 million arising from a rationalisation of its Biopharmaceuticals business unit, primarily relating to severance costs and the transfer of most pharmaceutical distribution activities and certain inventory to one location in the United States, resulting in exceptional inventory write-offs. The remaining exceptional charges primarily related to asset write-downs. For additional information regarding exceptional charges, please refer to Note 3 to the Consolidated Financial Statements. Net Interest and Other Expense ; Income Net interest and other expense was $50.4 million for 2001 as compared with net interest and other income of $88.6 million for 2000. Interest payable and other charges increased by 110% to $291.9 million for 2001 from $138.8 million for 2000, primarily reflecting interest payable of $40.3 million on the 7.25% senior notes due 2008 the "7.25% Senior Notes" ; , issued by Athena Neurosciences Finance, LLC "Athena Finance" ; , an indirect wholly owned subsidiary of Elan, in February 2001, interest payable of $35.4 million on the Series A, B and C senior guaranteed notes issued by EPIL III in March 2001, an increase of $21.1 million due to the inclusion for 2001 of a full year of interest payable on the 9.56% senior guaranteed notes due 2004 the "9.56% Guaranteed Notes" ; issued by EPIL II in June 2000 and increased financing and other fees. Elan expenses the subsequent funding it provides directly to business ventures. This is expensed within the interest and other expense line. Elan expensed approximately $24.6 million and $10.0 million for this subsequent funding, in 2001 and 2000, respectively. Income from financial assets increased by 6% to $241.5 million for 2001 from $227.4 million for 2000. Interest and other income increased to $159.2 million for 2001 from $112.5 million in 2000. Gain on financial assets decreased to $80.5 million in 2001 from $109.3 million in 2000. Gain on financial assets in 2001 includes $31.5 million for the sale of approximately 20% of Athena Diagnostics, Inc. "Athena Diagnostics" ; in December 2001. Foreign exchange gains amounted to $1.8 million in 2001 and $5.6 million in 2000. For additional information regarding indebtedness, please refer to Note 15 to the Consolidated Financial Statements and to "Debt Facilities" in this Financial Review.

Stages Sections 65 to 73 give the procedure to be adopted by the conciliator. Their gist can be stated in short form: The conciliator, when appointed, may request each party to submit a statement, setting out the general nature of the dispute and the points at issue. Copy is to be given to the other party. If necessary, the parties may be asked to submit further written statement and other evidence. The conciliator shall assist the parties "in an independent and impartial manner", in their attempt to reach an amicable settlement. The conciliator is guided by the principles of "objectivity, fairness and justice". He has to give consideration to: rights and obligations of the parties; trade usages; and circumstances surrounding the dispute, including previous business practices between the parties.50 He may, at any stage, propose a settlement, even orally, and without stating the reasons for the proposal.51 He may invite the parties for discussion ; or communicate with them jointly or separately.52 For successful conciliation, parties themselves must, in good faith, co-operate with the conciliator and supply the needed written material, provide evidence and attend meetings.53 If the conciliator finds that there exist "elements of a settlement which may be acceptable to the parties", then he shall formulate the terms of a possible settlement and submit the same to the parties for their observation. On receipt of the observations of the parties, the conciliator may re-formulate the terms of a possible settlement in the light of such observation. If ultimately a settlement is reached, then the parties may draw and sign a written settlement agreement. At their request, the conciliator can help them in drawing up the same.54 Legal Effect The settlement agreement signed by the parties shall be final and binding on the parties.55The agreement is to be authenticated by the conciliator. 56 The settlement agreement has the same status and effect as if it were an arbitral award rendered by the arbitral tribunal on agreed terms.57 The net result is that the settlement can be enforced as a decree of court by virtue of section 36 of the Act.

Challenge Funds have been established by DFID mainly within the field of private sector development to encourage private companies to extend their activities into new developmentally beneficial areas. Companies present proposals to such funds and those competitively selected generally receive funding to supplement the company's own investment. This ensures the commitment of the participating companies in a way which may not develop through simple competitive tendering. Challenge funds already exist in areas such as financial sector deepening and the development of business linkages challengefunds ; . For example, the DFID Business Linkages Challenge Fund in Ghana is supporting the franchising of 600 chemical sellers typically one-person private business operators who have acquired a licence from the Pharmacy Council PC ; of Ghana to sell retail Over the Counter OTC ; drugs ; , where the "franchising" involves improved and more cost-effective procurement and the standardisation of drugs. 16 PSI Crown Agents Bed Net Procurement: case study report September 2002 and doxycycline.

Prognostic factors in bronchial asthma in chiropractic practice. Nilsson N, Christiansen B. J of Australian Chiropractors' Assoc, 1988; 18: 85-87. In this study of 79 subjects, improvement was reported on average after five adjustments, after one month of care. By Andy Heller Staff Writer The functional redundancy of the body's immune system triggers the synthesis and release of proinflammatory mediators in response to irritation or injury to the body. While the system is normally responsible for restoring the body to homeostasis, it doesn't always hold up its end of the bargain, leading to the chronic and acute inflammation seen in autoimmune and inflammatory diseases. This is where Resolvyx Pharmaceuticals Inc. comes in, as the company is focused on developing compounds based on the endogenous resolvins that are produced in response to inflammation. Resolvyx Boston, Mass. ; was founded in 2003 and formally began operations in November 2005 after raising $12 million in a series A round led by CHL Medical Partners. CSO Per Gjorstrup, who says the company is alone in the space, believes using endogenous resolvins as a template to develop synthetic versions allows the company to take its molecules directly into disease models, skipping the step of hit-tolead chemistry, which can "cut a couple of years off of early stage development." Resolvyx has an exclusive license to a patent portfolio of endogenous resolvins and their analogs from Brigham and Women's Hospital, where co-founder Charles Serhan is a director. Resolvins are lipid mediators formed through the oxygenation of the omega3 fatty acids eicosapentaenoic acid EPA ; and docosahexaenoic acid DHA ; . According to Gjorstrup, recent data suggest that resolvins play a role in managing the immune response in preclinical models of colitis, periodontitis and arthritis by down-regulating TNF alpha, interleukin-1 IL-1 ; , IL-6, endothelial adhesion monocytes, IL-12, neutrophils, eosinophils, prostaglandins, leukotrienes, cyclooxygenase-2 COX-2 ; and induc, for example, crclor allergy.

European Union. As I said at the outset, the revised Treaty now recognises that employment is a matter for common concern at the level of the Union that is, it is recognised as an appropriate area to be co-ordinated at European level. In particular the new Employment Chapter gives the co-ordination process some bite it enables the Commission to propose employment policy guidelines for adoption by the Council of Ministers on a qualified majority basis. Furthermore, it puts in place a surveillance process whereby implementation of the Guidelines will be monitored and recommendations may be directed to the Member States with regard to their implementation performance. While the new Treaty does not yet have effect, the Prime Ministers, at the Amsterdam Council, mandated the immediate implementation of the provisions of the Employment Chapter including the preparation of Employment Policy Guidelines. It is these Guidelines that have been the main focus of attention in the run up to last week's Employment Summit. The Guidelines themselves are based on a four pillar framework aimed at: promoting employability through programmes aimed at reintegrating the long-term unemployed and young unemployed into the labour market, reducing early schoolleaving, strengthening apprenticeships, increasing investment in human resources, reducing indirect costs on labour, especially at the lower paid end of the market; promoting SME's and entrepreneurship through seeking to reduce administrative burdens, improving access to finance, making it easier and less costly to establish businesses, reducing the burden of taxation on labour; promoting adaptability essentially through the promotion of partnership at the level of the firm, strengthening support for training in business; and, promoting equal opportunities through facilitating increased participation by women in the labour market, for example, ceclor allergic.

Ologic functions of astronauts were adapted for sleep monitoring Figure 2 ; . Polysomnographic recordings using electrooculogram EOG ; , electromyogram EMG ; , and electroencephalogram EEG ; can be used to divide sleep into stages. In some ways, sleep staging is an artificial construct designed for analysis of sleep based on our available monitoring techniques. However, research has revealed that these sleep stages have physiologic and behavioral correlates that are clinically important. REM sleep occurs about every 90 minutes and is sometimes followed by short periods of waking. During REM sleep, low voltage, fast EEG activity is associated with rapid movements of the eyes and low EMG tone in most antigravity muscles. Non-REM NREM ; sleep is divided into stages 1 to 4. Stage 1 sleep is the transition from drowsy wake to sleep and is characterized by slow rolling eye movements and the disappearance of the EEG alpha rhythm. Stage 2, often the stage dominating much of the night, is light sleep, defined by the presence of sleep spindles and K complexes on the EEG. Stages 3 and 4, also known as deep sleep, include large amounts of the slow 1 Hz ; delta rhythm on the EEG. Sleep stages occur in cycles throughout the night.3, 21 Sleep-state alteration is frequently seen with psychoactive medication use. CNS active medications often alter the occurrence, latency, and EEG characteristics of specific sleep dream states, either with therapeutic intent or as side effects. Even some nonpharmacologic therapies.

The new techniques measured normal tissue levels of oxygen and hemoglobin, which carries oxygen in the blood. Breast cancer tissue is "more active" Poplack said, and uses more oxygen and blood to survive. Scientists also measured cell membrane structure and the tissue's ability to conduct and store electrical charges. Poplack's team used the alternative technologies on 23 white women ages 40 to 79 who had a history of normal mammograms, the study said. The five-year, $7 million study funded by the National Institutes of Health's National Cancer Institute is part of a larger effort to find alternative ways to create images of the breast, principal investigator Keith Paulsen said. The researchers, who are seeking another $10 million in National Cancer Institute grants, are working on a second study using the new techniques on women who have had abnormal mammograms. Poplack said another five years of intensive study was needed before broader studies could be done with more women, including those of different races. Commercial versions of the tests are at least 10 years away, he said. Barbara Croft, head of the Cancer Institute's Cancer Imaging Program, said there was "a big need" to find alternative ways to look inside the breast. Any test that is easier, more available, less.

Efforts offset, in part, by price decreases and the negative effects of exchange rate fluctuations. Major products contributing to this increase were Gemzar, Humulin, Permax and ReoPro. These sales increases were partially offset by declines in anti- infectives and Axid. Sales of Prozac outside the U.S. decreased slightly compared with the prior year. Increases in Prozac sales achieved in most countries around the world were offset by declines in certain countries, primarily Australia and Canada, due to generic competition, and France, due to competitive pressures. Worldwide sales of animal health products increased 7 percent, to $547 million. Sales increased 10 percent outside the U.S. and 3 percent in the U.S. The worldwide sales increase occurred across a majority of the product line. Cost of sales was 28.8 percent of sales for 1996 compared with 27.9 percent in 1995. This increase primarily reflects the impact of increased health-care-management revenues, which have lower margins than pharmaceuticals, and reduced production volumes of certain products as the company endeavors to reduce inventory levels which resulted in greater amounts of overhead costs being charged against income ; . These increases were partially offset by productivity improvements and an improved pharmaceutical sales mix. The company anticipates that cost of sales as a percent of sales may increase slightly in 1997 as reductions in costs as a percent of sales for the core pharmaceutical business will likely be more than offset by increases in revenues from health-care-management services, which have higher costs as a percent of revenues. Research and development expenses increased 14 percent in 1996. Expenses in support of global clinical trials, as well as an increase in external research collaborations relating to the discovery and development of new technologies, compounds and delivery systems, drove this increase. The company expects spending in research and development to increase approximately 14 to 17 percent during the next year. The actual increase may vary depending upon the level of research collaboration activity. Marketing and administrative expenses increased 7 percent in 1996. In the second quarter of 1996, the company implemented cost- containment programs designed to reduce the overall rate of expense growth while directing greater funding to new product launches and globalization efforts. These programs helped slow the rate of marketing and administrative expense growth to 4 percent for the last half of 1996 compared with 11 percent during the first six months. Overall, marketing and administrative expenses for 1996 increased largely due to costs associated with the launches of the company's new products, Gemzar, Humalog and Zyprexa; continued efforts to expand globally, especially in emerging markets; the development of enhanced information technology capabilities; and increased compensation accruals due to the company's performance- based bonus programs. In both 1993 and 1992, the company initiated various restructuring and streamlining initiatives and strategic actions. See Note 3 to the consolidated financial statements for a further discussion. During 1996, the company continued the implementation of these initiatives. Of these restructuring charges, approximately $33 million and $51 million were paid in cash in 1996 and 1995, respectively. Charges yet to be paid in cash total approximately $212 million and are expected to be funded from operations primarily over the next few years. Interest expense of $289 million in 1996 was approximately the same as in 1995. Net other income for 1996 amounted to $273 million, which was $203 million higher than in 1995. The increase was primarily the result of several nonrecurring items: the sale of the U.S. marketing rights of Ceclora CD and Keftaba to Dura Pharmaceuticals, Inc., in the third quarter for approximately $100 million; income received under royalty, codevelopment and comarketing contracts; the sale of marketing rights for ReoProa in Japan and Tapazolea in the U.S.; and gains from the sale of certain equity securities. The effective tax rate for 1996 was 25 percent compared with 26 percent in 1995. The decline is primarily the result of changes in the mix of earnings between jurisdictions having lower tax rates 2.
